When submitting this form, attach a picture of your photo ID.  If your photo ID does not have your current address, please attach a picture of one (1) additional proof of residency such as a recent utility bill, pay stub, lease agreement, or tax bill. 

After a staff member verifies your eligibility, we will mail your library card to you via the USPS. Your temporary card will be valid for one month. Please bring in your library card and photo ID before your library card expires, and we will complete the registration process and extend your expiration date for the full 2 years.
